Applying for a KIt

This form will ask you for your information and provide a list of items that we currently have access to (check all that apply).
What's inside
Each kit is customized by the person who puts in the request. There is a section at the end that asks for any suggestions of items not listed - if it is an item we have access to, we will do our best to secure it for you - please note that it is important to note the sizes and details of the gear you need. Once the kits are ready, we will send you an email when your kit is ready to coordinate a drop off. (Check your junk mail, if you don't respond, we can't get you your kit)
**Note: "Nude" and "skin coloured" are not universal and do not help us identify the "colour" gear you wish to receive. Using words like "Light, medium, dark" can better help us understand what it is you are looking for without reproducing the narrative of "white" as neutral/ nude in relation to skin tones.

We are trying our best to source tools and products, and receiving donations from generous community members. That said, we can't promise that your kit will include everything you have requested.
